New Patients for Primary Care
Please bring your completed new patient forms with you on your first visit. We look forward to helping you reach all of your healthcare goals.
Along with your new patient forms please bring the following:
- A list of all medications
- Names and addresses of other physicians you are seeing
- Any recent medical records or labs reports you have in your possession
- Insurance cards, Medicare cards, and a Picture ID
New Patients for Mental and Behavioral Health
Services are provided at both our physical location and via telehealth [ZOOM].
Please complete NEW PATIENT FORMS and follow the instruction page for returning. If you will be seen at our office location PLEASE BRING ALL FORMS WITH YOU along with a photo ID and insurance cards.
Medical Records Release
If you would like us to obtain your medical records from another provider, please complete our Authorization to Release Protected Health Information form. Bring the completed form to your appointment, or contact our office for return instructions. Authorizations expire 90 days after the date signed.
